Therapeutic application of tramadol in the treatment of pain compared to other opiates |

1. A case of 4th ventricular tumour is being presented, which confirms the concept of CTZ in man.
2. Vomiting in this case was unresponsive to triflupromazine, metoclopramide and promethazine. It was controlled by intravenous hydrocortisone within 6 hours and recured after stopping steroids.
3. This may be attributed to release of pressure of the tumour on CTZ because of anti-inflammatory effect of hydrocortisone.

Effect of antibiotics on neonatal polymorphonuclear leucocyte function in vitro |

1. A micromethod requiring a few drops of blood was used to measure simultaneously neutrophil phagocytosis and bactericidal activity in ten preterm neonates, ten full term normal healthy neonates and ten normal adult volunteers.
2. The in-vitro effect of three antibiotics erythromycin, gentamicin and chloramphenicol on the neutrophil phagocytic and bactericidal function was assessed after incubating the antibiotics with the cells from neonates and adults.
3. As opposed to adults, wherein erythromycin enhanced phagocytosis by the polymor-phonuclear leukocytes (PMNs), no such enhancement was observed with the PMNs from preterm and normal neonates.
4. chloramphenicol and gentamicin had no effect on the PMN phagocytic function in adults and neonates.
5. The bactericidal function of PMNs from adults and neonates was found to be unaltered by all three antibiotics.

Assessment of the tolerability of hydroxy-propyl methyl cellulose in two different vehicles in rabbit eyes. |

1. 2% solutions of locally manufactured Hydroxypropyl methyl cellulose [HPMC) in Balanced Salt Solution (BSS) or Ringer Lactate (RL) with or without a colouring agent were tested for tolerability in rabbit eyes, in four sets of 6 rabbits each.
2. Aqueous was aspirated and replaced in the left eye with HPMC solution, the right eye served as control. The eye was examined for inflammation daily for 10 days, while intraocular pressure (IOP) was measured on days 1, 2, 5 and 10.
3. Over 10 days observation period the eyes were clinically normal. There was no bacterial growth in aqueous on culture and no signs of inflammation on histopathology in eye on day 10.The IOP showed a significant fall, but the absolute difference in the means was only 0.5 mm of Hg. It is concluded that 2% HPMC in both BSS and RL with or without a colouring agent is well tolerated in the rabbit eye.

1. Degranulated mast cells were more in the peripheral region of Rat Mesenteric pieces while compared to their central region under normal conditions.
2. Toxic sod. chloride extract of epoxy polymer (caused deaths of Mice when injected intravenousely) produced degranulation of Mesenteric mast cells.
3. Degree of degranulation as a result of toxic epoxy polymer extract increase in concentration exposed.
4 Treatment with the saline extract of non-toxic epoxy material (That caused no mortality of Mice when injected intravenousely) did not show the effect of degranulation in Mast cell.
5. In conclusion the in vitro mast cell techniques offered an added advantage over the in vivo models in studying the toxic profile of biomaterial and thereby reduce the number of animals used.

Monitoring of phenobarbitone and incidence of adverse drug reaction of antiepileptic agents in Kashmiri population |

1. 62 patients who were receiving phenobarbitone either alone or in combination with phenytoin were monitored for serum phenobarbitone steady state level.
2. There was no statistical difference (p>0.05) in serum levels achieved with phenobarbitone alone (16.8ñ 5.3 'g/ml) and in combination with phenytoin (15.9ñ 7.5 'g/ml).
3. It was observed that patients on phenobarbitone alone and with drug levels above 10 'g/ml exhibited therapeutic response as compared with patients on combination. However, the patients with drug levels less than 10 'g/ml had therapeutic failure.
4. 28.8% patients showed Adverse Drug Reactions including sedation, behavioural problems, gum hypertrophy, skin eruption,cerebellar disfunction and digestive disorder.
5. Need to perform surveillance for Adverse Reactions and their relationship with dosage and plasma levels is empharized.

Incidence of adverse reactions to anti tubercular drugs |

1. 752 patients of tuberculosis were initiated therapy with 2, 3 or 4 drugs administered simultaneously in various combinations. Regular weekly checkup and relevant laboratory tests were carried out.
2. Streptomycin induced ADR's were observed in 50 (14.2%) patients. The commonest ADR encountered was giddiness (1 2%), which was unrelated to number of injections administered, but appeared early in higher age group.
3. Isoniazid caused ADRs in 37 (4.9%) patients.Peripheral neuropathy (2.1%) was the most frequently encountered ADR due to INH.
4. 9. 1% patients developed ADRs following thiacetazone administration. Gastric irritation was the commonest ADR (3.5%) followed by hepatotoxicity (1.16), jaundice (0.19%), skin rash (2.5%), Stevens Johnson syndrome (0.38%) and giddiness (0.5%). Ethambutol was found to be responsible for ADRs in 25 (7.3%) of patients. No impairment of colour vision was seen. Constriction of visual fields occurred in 5.5% and diminished visual acuity in 2%.
5. With the use of rifampicin, asymptomatic elevation of liver enzymes was observed in 6.85 and jaundice in 5.7% of the 175 patients treated with this drug.
6. Toxicity of streptomycin increased with age and was not related to sex or duration of the therapy. Reactions to isoniazid, ethambutol and thiacetazone were not related to age, sex or duration of therapy. However the incidence of ADRs was marginally higher among females following rifampicin.

Effect of verapamil on gastric ulcers in rats |

1. Verapamil, a calcium channel blocker has been found effective in reducing gastriculcers induced by pylorus ligation and aspirin in rats.
